Metatarsal presubluxation syndrome

I am a distance runner and am in the middle of "metatarsal pre0subluxation syndrome" and am hoping to be able to avoid surgery. I am have always been able to overcome all other injuries through patience, therapy, and rest. I have been unable to handle this injury.

Do any of you experts have any advice for me as to home-therapy? I am working with a great podiatrist who has prescribed custom orthotics and a toe splint for night.

Any input here is greatly appreciated.
