Elbow Pain while doing tricep exercises

I have been lifting heavy weights for several months now. I started feeling a slight pain in both elbows a few weeks ago. Now, I am at the point where it is very painful (in both elbows) to lift heavy weights, mainly while doing tricep exercises...some chest exercises hurt as well. I am trying to put on mass and lose excess fat. What can I do to alleviate the pain and get back to doing my tricep and chest exercises?
