shoulder injury

My wife was lifting waits this evening. while she was lifting she felt like she had a charlie horse in her left shoulder down into her bicep. After a few minutes it didnt bother her so she continued her work out. Tonight she is extreme pain and can barely move her arm. I have her icing it right now as well as have given her a muscle relaxer. Any idea what this might be and should she go to the doctor. Thanks for your help.
