break/dislocation: knuckle or finger

I landed hard on my fist (pinky knuckle) after a fall off a porch. My fist hit hard onto a stepping stone, and now it's swollen and red. My middle knuckle is large with soft swelling all around the topside of my hand. It's hard to straighten this finger all of the way back out. I can still work with the hand, but working with it increases the swelling/pain somewhat. Is there any chance of this healing naturally? What is the likely prognosis if this goes untreated?
