strained back rib? help!

2cd or third rib from the bottom, left side. Stop dead spasms with slightest twist, forward lead and certain extensions of the arms. (Not to mention laughs, coughs, slump ...) Some difficulty breathing--acute during a spasm. Swelling but no protrusion or discoloration. 50 years old, moderately athletic. 5'10" 178. Happened before but short duration and not quite so severe. What is it and what should I do?
