Allow injured athlete to play?

I'm researching the topic of when it is ethically reasonable to allow an injured athlete to play.
I have the following questions:

How do you determine when an athlete is "healed" enough to perform?
Would you allow an injured athlete to participate in an event if it is a "big" game or his/her last year?
What is the difference between being hurt or being injured?
Is it sufficient enough to just tape or use a compression sleeve on injuries during practice/competitions to prevent sitting out of an event?
When do you take into consideration the athlete's perspective on his/her injury? (ex. I can just push through the pain!)
I would just like the opinion of an athletic trainer.
