Pain under left shoulder blade

I have always had pain under my left shoulder blade off and on for as long as I can remember. It seems to worsen with stress. It is a very dull ache type feeling. Recently I noticed is has gotten almost unbearable especially when taking a very deep breath. I have no idea what is causing this. I do work out every day but not very aggressively. I do shoulder exercises once a week and maybe aggravated something, but the pain is not subsiding after one week. Any suggestions? I don't want to overlook something that may be underlying or serious.
