Pinched nerve? Arm pain

About a year ago, I started having pain in my upper LEFT arm after reaching quickly across my body to catch an object falling from the table. (I am right-handed.) The pain was intense, but sporadic, only occuring when I extended my arm in certain positions. Gradually, the pain and discomfort in the arm became more frequent. Now it wakens me from sleep 2-6 times per night. The arm always feels better during the day, though sometimes it will "tingle" almost constantly, and although my arm strength seems fine, it seems more difficult to hold things like a coffee cup in my left hand. I have some bulging on the inside of my left elbow, and some tenderness on my upper left back near the shoulder blade next to the spine. The upper arm is often tender to the touch, and still can cause excruciating pain at times. I have had X-rays and an MRI of my shoulder, both of which showed nothing. Also had 2 visits to a chiropractor who "adjusted" my neck only, and I quit, frustrated. What could be going on?
