Back pain when laying, sitting, standing, walking.

I have been having chronic back pain for years, and this week, it started again. When I lay down, it hurts. When I sit down, it hurts. When I stand up, same thing. Walking feels as if my spine is grinding together above my buttocks. On a scale of 1-10, my pain fluctuates between 5 and 9, 9 as the most severe. Doing anything is a chore, knowing that I'm getting ready to have serious pain by moving in any way. It subsides in about three weeks completely, but will be back in about 2-4 months. What causes this, and what can I do?
