bilateral calf pain

My son has been experiencing calf pain in both legs when he runs since December. He had a negative EMG and negative Doppler. If he persists in running the pain is so excrutiating that he limps and cannot continue. The last flare up caused pain that lasted weeks even though he was no longer running. He has been in physical therapy for 4 weeks and is pain free now as long as he does not run. As soon as he runs, even just going up the stair, he feels the pain. Any thoughts?
